Medicare for Married Couples: What Spouses Need to Handle Separately

Medicare does not sell family or couple plans. Each spouse has a separate Medicare number, enrollment date, plan choice and premium.

Spouses can choose different coverage

One spouse can use Original Medicare, Part D and Medigap while the other uses Medicare Advantage. The correct answer can differ because doctors, prescriptions, travel and risk tolerance differ.

One spouse’s job can affect the other’s timing

Coverage based on a spouse’s current employment may permit a delay of Part B without penalty. Confirm with the employer benefits administrator and Medicare. COBRA and retiree coverage do not extend the Part B Special Enrollment Period in the same way. The eight-month clock generally starts when current employment or job-based coverage ends, whichever happens first.

Work credits can help with Part A

A spouse may qualify for premium-free Part A using the other spouse’s Medicare-covered work history. That does not create shared Part B or a family Advantage plan.

Household income can affect both premiums

IRMAA is based on tax filing status and modified adjusted gross income. A married couple above an applicable threshold can face separate surcharges for each spouse. A Roth conversion or large gain may affect Medicare premiums two years later.

A five-question couple checklist

  1. When does each spouse turn 65?
  2. Whose current employment provides coverage, and when will it end?
  3. Which doctors and drugs does each person need?
  4. Could either spouse’s plan choice disrupt an HSA contribution?
  5. What happens to coverage and household income after death or divorce?
